Communication, or the lack thereof
Unless one is building a Web site for oneself, site development requires that stakeholders communicate among themselves. And it is this very communication that can cause a project to fail.
Communication requires that all parties understand and agree to what is to be accomplished, by when, and at what expense. It requires that a person articulate what they are thinking, and that the listeners (or readers, if the communication is in written form) understand what meaning was intended by the communicator. Said another way, did everyone understand, not just hear, what the speaker said?
